What are anti-vax people talking about with the inserts and whatnot?

The package insert lists the ingredients that are found in vaccines and the adverse events that have been reported for people that received the ingredients.

This gives you an idea what is injected in your body and what effects the treatment could have on you and what efficacy and safety studies had been performed.

Here is an example, the package inserts of a Hep B vaccine[1][2] :

First one can check the ingredients:

· 10 micrograms Hepatitis B surface antigen

· 0.25 milligrams Aluminum Hydroxide

· Yeast cells (Saccharomyces cerevisiae)

· Sodium phosphate

· Dibasic dihydrate

· Sodium phosphate

· Monobasic dihydrate

· Sodium chloride

· Polysorbate 20.

· Bovine derived materials

Then one can check the adverse reaction studies:

We find that over 5000 healthy adults and children were monitored for 4 days post-administration.

Checking the post-marketing experience section one can find a list of reported adverse events or events that have a suspected causal connection to the vaccine. It’s not always possible to establish a causal relationship to the vaccine as the science that investigates adverse events has often not been done.

we find the following adverse events:

Herpes zoster, meningitis, Thrombocytopenia, Allergic reaction, anaphylactoid reaction, anaphylaxis, Encephalitis, encephalopathy, migraine, multiple sclerosis, neuritis, neuropathy, hypoesthesia, paresthesia, Guillain-Barré syndrome and Bell’s palsy, optic neuritis, paralysis, paresis, seizures, syncope, transverse myelitis, Conjunctivitis, keratitis, visual disturbances, Earache, tinnitus, vertigo, Palpitations, tachycardia, Vasculitis, Apnea, bronchospasm including asthma-like symptoms, Dyspepsia, Alopecia, angioedema, eczema, erythema multiforme including Stevens-Johnson syndrome, erythema nodosum, lichen planus, purpura, Arthritis, muscular weakness, Injection site reaction, Abnormal liver function tests,

What can one learn from this information?

First, we do know that this vaccine contains 250 micrograms(0.25mg) of aluminum.

That number alone doesn’t tell us much. We get exposed to aluminum on a daily basis, it’s found in food and in the environment, we need to understand whether or not it is a small or a large amount. We need to compare it to other sources of exposure to get a better idea.

Since this vaccine is given to newborn babies we can look at the amount babies are exposed to and compare it to the amount the vaccine contains.

We do know that in the first 6 months of life a baby will ingest approximately 7000 micrograms of aluminum. The intestinal absorption of aluminum is very low ( ~ 0.3%) and we know that most of what is in a vaccine will be absorbed.

A simple calculation shows that in the first 6 months a baby will absorb 7000/300 = 23 micrograms of aluminum from breast milk and 250 micrograms from a single Hep B injection.

That’s 60 months worth of aluminum in one single shot.

What about the other ingredients? We do know that it could be possible that vaccination could sensitize the recipient not only against the target disease but also against other vaccine ingredients.

So we could see sensitization against bovine derived materials, yeast proteins and other ingredients and possibly proteins that are close enough to these antigens(molecular mimicry). For example it was speculated that the 2009 swine flu vaccine antigen for H1N1 caused the immune system to cross react with hypocretin which lead to the destruction of hypocretin secreting neurons in brain, the cells responsible for promoting wakefulness and this led to cases of narcolepsy.

The adverse events section shows us that the adverse reaction studies for this vaccine tend to be short-term and often last a few days only.

If we want to know about rare and possible long-term effects(narcolepsy would be one) we need to look at the list of reported adverse events. The problem is that this list tells us little about the actual rate because most adverse events are never reported and we can’t assume all events to be causal. That being said the events listed are more likely to be associated with the vaccine, not all reported events are listed.
